Temporary Part Time (approximately 4 months)

Reporting to the Manager, Western York Region OHT, the System Navigator will be responsible for supporting the Western York Region Ontario Health Team (WYR OHT) through the delivery of specific planning deliverables related to the adoption of a standardized navigation system and other key priorities. The Systems Navigator will focus on 24/7 patient navigation services for the WYR OHT’s attributed populations and will work in collaboration with other OHTs, regional partners and information and referral agencies to determine organizational readiness to co-design and adopt new navigation workflows, building off a current state analysis completed in year 1. The certified Systems Navigator (24/7 Patient Access and Navigation) position provides an opportunity to address real and systemic barriers to services for year 1 priority populations within the Western York Region catchment area. The position will support efforts to ensure equitable access to health and social services through overall support, project management and coordination of the Phase 1 implementation of a standardized navigation system for Western York Region Ontario Health Team (OHT). The System Navigator will provide system navigation and project management expertise for our 24/7 Patient Access and Navigation working group and project management leads to ensure successful completion of deliverables in collaboration with internal and external stakeholders. Contribute to safe, quality care by
  • Collaborate with WYR OHT partners and information and referral agencies to support
    • Collection of client experience and service utilization data
    • Co-design with OHT partners on how centralized access supports would connect to the provincial Health Care Navigation Service or Health Connect Ontario (HCO)
    • Strategic planning discussions with Ontario Health regarding validation of business requirements for HCO.
  • Participate in regular regional OHT-Navigation Service advisory committee meeting
  • Work closely with the Chair/Co-Chairs of the WYR OHT 24/7 Patient Access and Navigation Working Group.
  • Manage and update a navigation service utilization dashboard for participating navigation organizations for Phase 1.
  • Working with the cQIP Mental Health & Addictions (MHA) Working Group to identify, develop and implement a mental health and addictions support resource library to be housed in phase II of the online asset inventory.
  • Facilitate meetings and workshops with internal and external stakeholders to co-develop standardized navigation processes.
  • Coordination of the 24/7 Patient Access and Navigation Working Group, and management of the project’s goals, deadlines, and tasks.
  • Collaborate with members of the 24/7 Patient Access and Navigation Working Group and cQIP MHA working group through detailed data collection and analysis, consulting with participating organizations to support their overall meeting of expected goals and outcomes and drafting reports for submission to WYR OHT leadership.
  • Provision of project management support, as determined, to other OHT initiatives.
  • Build and maintain relationships with OHT partner organizations and external agencies.
  • Maintain asset inventory by liaising with community groups and other health/social service agencies to collect and document referral information.
  • Participate and lead working group committees as assigned.
  • Handle other duties as assigned.
What must you have?
  • University degree in health or social sciences field; or college diploma with two to five years work experience in the health and/or social work field. Work experience in elderly or mental health and addictions care would be preferred.
  • Minimum of 2 years of project management experience, ideally in healthcare environment such as community sector, hospitals, long term care, government, or private sector

What else do you bring?

  • Adept at identifying resources through the internet and networking with community agencies
  • Knowledge of various legislations ex. OW/ODSP, CPP-D, CFSA, WSIB
  • In depth knowledge of Ontario’s OHT model and local OHT structure/plan, a definite asset
  • Comfortable with technology solutions and virtual navigation systems
  • Knowledge of community and agency programs in Western York Region including services for frail and elderly adults and individuals requiring mental health and addictions support.
  • Strong knowledge of social services sector
  • 2-5 years of professional experience (or equivalent) working with patients and community agencies
  • Proven track record of being able to deliver on organizational targets
  • Demonstrated ability to work cross- collaborative with project partners to align approaches with other areas of work
  • Strong communication, interpersonal skills and time management skills
  • Microsoft Office, Microsoft Teams
  • Proven attendance record.
  • Ability to perform the essential duties of the job.
